Community Behavioral Health offers services on the Eastern Shore of Maryland and Howard County.
We also offer PRP and Medical Management from our expert multi-disciplinary team. 

Professional Referrals: If you are making a referral from outside the agency, please fill out the forms below and send a fax (888-509-0010) of the biopsychosocial, psychiatric evaluation, current/previous treatment plans, recent physical exam and immunization records.
